Moirai Ref

She has a name Moirai was the name of the fate witches in greek mythology (well one of the groups, there was another called Graeae)

Basically her story (right now, may change it later) was that in the 1500's, (medieval time period) she was a witch who would murder children, trapping their souls in hand made dolls inside her hut. The dolls would be made from the skin and hair of the child she killed, helping hold onto the essence of the soul. The souls fueled her power and dark magic. 

Later surrounding villages summoned a monk, who fought the witch, capturing her. The villages burned down her hut and dolls, freeing the souls of the captured children, draining the witches power.

When she was captured, the monk preformed a ritual on her, hoping to prevent her spirit's return. He chopped of one of her paw's as a trophey to prove the death of the creature. Removed a wing so the spirit could not fly away, one eye gouged out, the other a button sown over in mockery of her dolls. Her ears removed so she could not spy on the village, her mouth stitched together to not speak curses in the afterlife. Her chest cut open and her heart removed it's darkness could not taint the land surrounding it.

But before being killed, the witch with her last ounce of power, she sent her soul into a hidden doll the villagers missed, hidden among the roots of the tree's surrounding the remains of her hut. If the doll were ever to be destroyed, her should would be freed, and she'd reclaim her dismember body and seek revenge on those who destroyed her dolls and made a mockery of her body.
